2) Please use ONLY this form when applying. Submitting this application, in no way, guarantees that a scholarship will
be granted. There is no appeal, and the decision of the selection committee is final. Only U.S. citizens may apply.

*Use Delivery Confirmation from the U.S. Postal Service or enclose a self-addressed, stamped postcard if
you wish to get notice that your application has been received. Mail early to ensure your application is
received before the deadline.

4) Send letters of recommendation and an official transcript in sealed envelopes with the application package. It is acceptable to send the transcript separately if your school insists, but indicate on page 1 of the application that it will arrive separately.

5) Enclose check or money order for $10 payable to Carpe Diem Foundation of Illinois to cover processing costs. Please read our Common Questions page online for further explanation.
